The Home Enteral Nutrition (HEN) program is a state initiative allowing eligible participants to access Blended by Sarah products when discharged from a public hospital in NSW.
Healthcare professionals please register patients by completing our registration and order form.
NSW HEN Frequently Asked Questions
No. The NSW HEN program is linked with the NSW state health tender, therefore can only be accessed by patients being discharged from a public hospital in NSW.
Health Care Professionals can register participants by completing the HEN registration form and emailing through to our team. We will then be in touch with the instructions to order.
After the HEN Order Form has been received, our team will contact the participant directly to provide a schedule of costs.
Please contact your dietitian or nurse so that they can review your nutritional needs. They will submit a new HEN Order Form if you require a product change.
