HEN Program

The Home Enteral Nutrition (HEN) program gives eligible individuals access to Blended by Sarah products through a funded pathway, supported by state health agreements.

We are proud to hold health contracts with both the New South Wales and Northern Territory Health Departments, making our blends available within hospital settings and through community HEN programs in both states.

NSW HEN Frequently Asked Questions

No. The NSW HEN program is linked with the NSW state health tender, therefore can only be accessed by patients being discharged from a public hospital in NSW.

Health Care Professionals can register participants by completing the HEN registration form and emailing through to our team. We will then be in touch with the instructions to order.

After the HEN Order Form has been received, our team will contact the participant directly to provide a schedule of costs.

Please contact your dietitian or nurse so that they can review your nutritional needs. They will submit a new HEN Order Form if you require a product change.

NT HEN Frequently Asked Questions

Eligibility is determined by the NT Health Department. Participants are typically individuals with a clinical need for enteral nutrition support who are being transitioned into community care. Please speak with your treating dietitian or healthcare team to confirm eligibility.

Healthcare professionals can register patients by emailing our team to request the NT HEN registration form. Once received and processed, we will follow up with ordering instructions.

A schedule of costs will be provided to the participant directly once the registration form has been received by our team.
